  • Disertai Surat Pengantar dari pengelola kepegawaian.
  • Untuk KPE hilang atau tertelan ATM agar segera melapor kepada Bank pemberi layanan.
Mekanisme Perbaikan Konversi NIP
  1. Beda Nama / Tgl. Lahir / Jenis Kelamin / Tmt. CPNS.
    1. Fotocopy SK CPNS.
    2. Fotocopy SK PNS.
    3. Fotocopy SK Pangkat Terakhir.
    4. Fotocopy Ijasah Terakhir.
    5. Fotocopy Konversi NIP yg Salah.
  2. Hilang
    1. Poin 1 – 5 di atas.
    2. Surat Keterangan Kepolisian.
Catatan :
  • Masing-masing persyaratan sebanyak 3 lembar.
  • Disertai Surat Pengantar dari Pimpinan SKPD.
